Step 1
Please pay attention to the instructions since they ask for specific marks.
On the “AGENCY TYPE” – Click applicable boxes and provide additional details as required.
On the “SYSTEM ACCESS REQUEST” – Specify (A) for Add, (D) for Delete, or leave blank in space provided.
Step 2
Sign, Scan and Forward to Stephen Jelinek. He will submit your request to AOC
Step 3
There will be an undefined wait time for the AOC to process the request. It can be from hours to days. We cannot expedite or control their process.
